Adverse Event Reporting
Submit reports of adverse events from animal foods or health products as well as suspected failures of animal health products to the manufacturer and/or one of the following:

Animal Blood Bank hotline—800-243-5759
A 24-hour hotline that focuses on transfusion medicine (particularly blood component therapy), recommending dosages and infusion rates, at no cost to caller.

ASPCA National Animal Poison Control Center (allied agency of the University of Illinois College of Veterinary Medicine)—888-426-4435
A fee of $60 or $55 dollars for veterinarians signed up for our Veterinary Life Line Partner program is charged per case, with no extra charge for follow-up calls. There is no charge when the call involves a product covered by the ASPCA Animal Product Safety Service.

Eastern Veterinary Blood Bank—800-949-EVBB (800-949-3822)
A 24-hour, commercial blood bank that focuses on transfusion medicine.

DEA Toll-Free number for Office of Diversion Control, registration section—800-882-9539.

FDA/CVM: Drugs, Devices, Animal Feeds—888-FDA-VETS (888-332-8387)
Calls to report suspected adverse drug experiences will be forwarded to the CVM's Division of Epidemiology and Surveillance; voice-mail messages can be left during nonbusiness hours.
FDA/CVM shortage coordinator—240-276-9207
Call to report shortages of medically necessary veterinary drugs.

Food Animal Residue Avoidance Databank—888-USFARAD (888-873-2723)
